Job Description

Position Description:

Plans, organizes and conducts dietetic education and nutritional counseling.

Essential Duties and Responsibilities:

  • Serve in a consultant role or per diem employee;
  • Consult with patients on nutritional needs;
  • Participate in Interdisciplinary Group meetings;
  • Virtually assess patient records through an electronic medical record;
  • Coordinate nutritional plan of care with Registered Nurse and Medical Director;
  • Plan, develop, implement and maintain programs and systems of nutritional care;
  • Ensure nutritional care will meet the needs and maximize the quality of life for hospice patients;
  • Ensure nutritional needs are met and documented to fulfill the requirements of all federal, state and local regulations;
  • Develop and implement a nutritional plan of care based on assessment of nutritional needs as part of the patient’s total plan of care;
  • Provide instruction of in-service education to both staff and patients and their families;
  • Maintain effective relationships with all team members, patients and families;
  • Attend all mandatory in-services, meetings, and training sessions as required;
  • Comply with organizational policies and procedures and abides by all HIPAA laws, Medicare/Medicaid and DHEC Regulations as well as Conditions of Participations (CoPs) to maintain patient rights, privacy and due care; and
  • Promote the organization’s mission and company policies.

Working Conditions/Physical Requirements:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Prolonged standing and walking required. Requires working under some stressful conditions to meet deadlines and patient needs. May require visits in unsafe neighborhoods and dwellings that may not have elevators.

Job Qualifications:

Education: Graduate of an accredited college with a BA Degree in Foods, Nutrition Food Services or Institution Management or equivalent coursework.


Licensure: Current State Licensure as a Dietitian or Nutritionist.


Licensure: Registered by the Commission on Dietetic Registration of the American Dietetics’ Association.


Licensure: Current Divers License applicable to the state of practice


Transportation: Reliable transportation with valid and current auto liability insurance. Will need to travel locally with some exposure to weather.


Licensed in multi-states (specifically GA, MS, SC)



Geriatric experience


Hospice experience


Skills and Abilities:

Ability to communicate well in English (written and spoken).


Indirect supervision of other department if necessary.


Computer skills


Ability to utilize PPE when potential contact with blood or body fluids is possible. Follow infection control guidelines.


Must demonstrate effective communication skills, both verbal and written.


Ability to critically think.



Must have knowledge of and ability to utilize clinical and administrative dietetics in persons with terminal illness.


Infection control measures and use of PPE.

Participation in Agency Quality Assurance Program.


Provide required support, skills, and documentation at time of death.


Ability to adapt to Agency software for documentation.


Competency in taking accurate patient referrals and forwarding them to the appropriate person(s).

Organize your travel by routing patients so you drive the shortest possible distance between visits.

Registered Nurse PRN – Hospice: Edgefield, McCormick, Aiken

Job Description

Palladium Hospice & Palliative Care is seeking a PRN nurse to assist in our Edgefield, McCormick, Aiken SC region. Palladium is a multi-state fast growing Hospice provider. We offer flexible schedules & supportive environment. Come see what we have to offer.

Job Summary:

The registered nurse – plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. The Hospice RN utilizes the nursing process (assess, plan, implement, evaluate) to assume primary responsibility for the care of terminally ill patients and their caregivers in consultation with the primary physician, Hospice Medical Director and the Hospice interdisciplinary team.

Purpose of Role:

The RN evaluates need and coordinates care from other disciplines including Clinical Care Partners, Social Workers, Chaplains, Bereavement Coordinators and Volunteers. The hospice nurse provides physical and psycho-social support to the patient and supportive care to the caregiver and family that will assist them in accepting and preparing for the death of the patient. RN Case Manager reports to the Hospice Nurse Manager. Use of personal vehicle required.


For Palladium Hospice and Palliative Care, LLC to recruit and retain registered nurses to support the staffing needs, maintain employee satisfaction and meet the needs of patients, families and the organization.

This policy applies to all Registered Nurses employed with the status of PRN.

A. Supervisors are responsible for planning for staffing needs based on organization standards and patient care needs.

B. A commitment is required to be available to work a minimum of the following:

  • Two weekdays/month; or
  • Two weeknights/month on call: or
  • One weekend on call a month

C. As needed (PRN) status does not guarantee work hours, but the employee will be committed to be available on the schedule and acknowledges it will be based on the need.

D. Schedules must be provided to the immediate supervisor on a consistent basis,

E. PRN registered nurses are eligible for on-call incentives if scheduled to be on call, regardless if productive hours are documented.

F. At any time the PRN registered nurse desires to select a different tier, the supervisor must approve and it will be based on need. A new acknowledgement form must be submitted to HR.

G. PRN employees must meet all the mandatory and ongoing requirements for compliance with the organizational policies.
