We are devoted to ensuring our veteran’s program is successful.  As a level 3 partner of the National Hospice and Palliative Care Organization (NHPCO) we have built strong relationships with the Veteran’s Affairs (VA) offices across South Carolina, Georgia and Mississippi as well as organizations such as the American Legion and VFW.

We know what they sacrificed for our country and for each and every one of us.  Some of us have been lucky enough to have heard their stories as they impart wisdom on us unknowingly.  We now have this rare opportunity to repay them in this small way. Give us the chance to be their hero, just as they have been yours for so many years.

Our veteran’s program is designed to focus on the social and emotional needs of the patient and their family members.  Each veteran is paired with a social worker that will work to uncover the patients’ needs and develop an individualized plan of care.  The social worker will help alleviate stress by reaching out to Veteran’s Affairs offices, gather information on benefits and fill out any necessary paperwork.  Educating the patient and their family members on specific benefits and resources is just another one of many ways your social work can offer support.

What is Hospice?

Hospice is a holistic and compassionate service committed to assisting patients and caregivers facing a life limiting illness. Our mission embraces the physical, social, emotional, and spiritual needs of those we serve using a comprehensive team approach.

Who Qualifies for Hospice Care?

Patients who have a medical prognosis of life expectancy of six months or less, if the illness runs its normal course, are eligible for hospice care. A physician must make this determination. There is no end to the hospice benefit under Medicare if the patient continues to meet the “expected within the next 6 months” criteria.

Where is Hospice Available?

We coordinate hospice care with your family and caregivers in the comfort of your home, an assisted living facility or skilled nursing facility.

Who Pays for Hospice Services?

All care provided by the hospice team is covered by the Medicare Part A benefit as well as most private insurers, Medicaid and Veteran’s benefits. This includes:

  • Physician, nurses, aides, chaplains, social workers, volunteers and bereavement support.
  • Medication that is related to the hospice diagnosis – Medical equipment (bed, wheelchair, lift, etc.) – Medical supplies (briefs, mattress covers, wipes, etc.)
  • No patient is denied services due to an inability to pay.
